Default Contrast Enhanced PET/CT Billing

Would anyone be willing to share their experience in billing options when administering IV contrast during PET/CT exams? I have heard that you need to bill a 59 modifier for the contrast enhanced portion(Medicare) since we are already billing/performing the PET/CT on the same day. I have also heard contradictory information that you may bill the diagnostic CT code as well as the PET/CT code. Anybody out there have any real time experience with this?
